Intended for existing clients needing follow-up care for psychiatric medication(s), supportive counseling, and health education
Length of Time: 1 Hour

Ideal for clients who would like to participate in Ketamine Assisted Psychotherapy with their therapist who offers Ketamine Assisted Psychotherapy services. This service also includes an initial psychiatric evaluation, psychotherapy, and psychiatric medication evaluation.
Initial Appointment Length of Time: 2 Hours
Initial Appointment Cost: $620
Follow Up Appointment Length of Time: 1 Hour
Follow Up Appointment Cost: $310

Ideal for clients who have treatment-resistant depression, have unresolved trauma that is causing anxiety/depression/irritability, no longer want to be on medication for their anxiety/depression, or do not want to be on medication for their anxiety/depression and would like to instead heal inward with ketamine which has the ability to heal anxiety/depression/trauma without needing daily medication.
Length of Time: 3 Hours
Cost: $930

Indended for clients 1-3 days after their Ketamine Assisted Psychotherapy session with me to discuss outcomes of their session and how to integrate thoughts, feelings, and actions moving forward in order to make sizeable changes to overall mental health.
Length of Time: 1 Hour

I bill for the following services at $360.00/hour pro-rated in 5-minute increments:

Clinical Telephone Consultations: emergency, after hour, clinical question, and scheduled phone check-in calls. Charges will apply for phone time over 10 minutes, or for time over 5 minutes when there is more than one call per week.

Insurance support, including prior authorizations for prescriptions, disability Insurance and FMLA paperwork, copying chart notes, etc.: no charge for the first 10 minutes, then standard rates apply.

Legal and Legal Expert Services: report preparation, letters to courts, conferences with attorneys, and legal testimony including travel time.
2. Home visits may be subject to a service fee for drive time. This will be discussed prior to your appointment if this service fee applies to you.
3. There is no charge for appointment scheduling or administrative calls or for routinely refilling prescriptions
4. I charge for all late-canceled and missed appointments (no exceptions):

Less than 24 hours notice: full fee

Less than 48 hours but more than 24 hours: ½ fee

Less than 72 hours but more than 48 hours: ¼ fee
